How to book your consultation:

Step 1: Obtain a signed requisition from your referring doctor. As an OHIP-covered service, cardiology consultations require a signed requisition from a registered Ontario physician.

  • Need a signed requisition? Download our requisition form and visit your family doctor. If you do not have a family doctor, visit MyDoctor Now. They will conduct a same-day, OHIP-covered telephone assessment to determine whether or not you could benefit from a cardiology consultation.

Step 2: Ask your doctor to E-mail your signed requisition to contact@cardiologynow.ca  or fax it to 1-800-341-9514.

Step 3: We will call you to confirm your appointment with a day and time that is convenient for you. If you have questions about your appointment, or if you would like to reschedule, please call  1-833-904-4555.

An online or telephone consultation can be conducted using secure approved technologies that enable video conferencing or by telephone. The actual platform for the initial consultation will be determined in advance by our cardiology team and depends largely on the medical situation.

Recent research has shown that online or telephone consultations lead to better communication with patients in terms of educating patients and also allow for less stressful encounters. Moreover, it has shown to be effective and beneficial in allowing for early detection and treatment, especially in rural communities with limited access to cardiologists.

This service is for adult cardiology consultations and testing.
You can verify the status of their license and view their credentials on college of physicians and surgeons of Ontario (CPSO).
Recent bloodwork is required for our cardiologist to conduct a full assessment.

You can go to any laboratory that is convenient to you. Please have this done at least 4 business days before your cardiology consultation.

Before setting up your online or telephone consultation session you will need:

A referral from your family physician, walk-in clinic or nurse practitioner.
Valid OHIP card
Verbal consent for the upcoming visit will be obtained.
An email address or cellphone to receive appointment related information.
Pharmacy contact details, along with a list of your medications
A translator if you are unable to communicate fluently in English. Any support person would be welcomed to accompany you during the consultation. We do have physicians that speak various languages. Feel free to request at the time of booking.

Upon receipt of a referral, our cardiology team will call you to book your online or telephone consultation. You will be asked a series of questions to further determine your care needs and identify the technologies you have access to. Please ensure you are in a quiet place for the appointment.

For consultations by telephone, a 2hour window will be provided to you for when our cardiologist will call. Please anticipate a phone call from a number you may not recognize as this call will be coming directly from Dr. [Last Name].”

For consultations by video conference, patients must have an email address and computer or a smartphone with a camera.

If you have access to the internet you will be required to complete a patient form that will include a list your medication, family history etc. prior to your appointment.

All our services are covered by OHIP and some private insurance companies.

This information is required for our cardiologist/clinic to request the appropriate prescriptions or access your medication history, as required, prior or after your consultation.

Our cardiologist will discuss your current health status, any symptoms you are experiencing and your needs. If required and deemed necessary by the cardiologist, they will arrange for an in-person visit and discuss options and next steps with you.

Appointment lengths vary depending on situation they can range from 15 minutes to 30 minutes.
Our goal is to complete your report and delivered to your family doctor within 5 business days but actual delivery time my vary. Please follow-up with your family doctor accordingly.
Appointments are booked within a couple of days upon receipt of your referral from your family doctor or healthcare provider. If you have not heard back from our clinic within 2 business days, then please call our clinic @ 1-855-680-3111.
A referral from a walk-in clinic or a nurse practitioner is acceptable.

We do not do prescription repeats. Please contact your family physician for this.

Online or telephone care has some inherent privacy and security risks that your health information may be intercepted or unintentionally disclosed. In order to improve privacy and confidentiality, you should also take steps to participate in this online or telephone care encounter in a private setting and should not use an employer’s or someone else’s computer/device as they may be able to access your information.

Cardiology Now is not responsible for the security of patients’ internet service providers, email domains, personal devices, or personal computers. Please only use your personal email when communicating your health information with Cardiology Now.

If it is determined that you require a physical exam you may still need to be assessed in person. You should also understand that online or telephone care is not a substitute for attending the Emergency Department if urgent care is needed.

If you are experiencing an urgent medical issue, please do not email to ask questions or report your symptoms, call 911 or visit an Emergency Department or Urgent Care Centre.
